Be prepared to submit the following information to claim your rebate of up to $60 of the cost of one box of qualifying factory loaded ammunition or bullets for reloading (tax and shipping not included).

1. Your contact information, including mailing address and email address.
2. Name of store where your box of non-lead ammunition was purchased.
3. Upload a copy (image or PDF) of your receipt for qualifying non-lead ammunition. Purchase date must be between 1/1/2021 and 1/8/2024.
4. Upload a photo of the UPC bar code on the ammunition packaging.

   (Your photos may turn out best if you do not use the flash on your camera.)

5. Upload photo of ammunition box showing product name and caliber (often on the side of the box).
6. Upload a copy (image or PDF) of your Deer Management Permit (DMP) showing it is for one of the following New York State Wildlife Management Units: 3H, 4F, 4G, 4H, 4O, 4P, 4R, 4W, or 6G. If you do not have a DMP but plan on trying to fill your buck tag in one of the qualifying WMUs listed above, upload a copy of your hunting license.

By submitting your rebate request you agree to be contacted for a follow-up survey.

Your rebate will be mailed to you in the form of a pre-paid Visa® card.

If you prefer to mail your rebate request via the U.S. Post Office, you can download and print this form.

Limit one rebate per individual. Rebate form must be submitted or postmarked by 1/8/2024.
